How to Submit a Preliminary Incident via Mobile

Overview

Users submit a Preliminary Incident Report Form when they identify an incident in the field. Examples of the Types of Incidents that are to be reported include Fire, Near Miss, Unexpected Release, or Vehicle Accident. Based on the Type of Incident that is reported, follow-up tasks will be assigned to the appropriate users.

Directions

  1. Log into the product using your log in credentials. (see getting started on the mobile app for instructions on how to save your password)
Log into the app
  1. Navigate to the Form Section on the App and click on Operations.
Click on Incident Reporting
  1. Open the Form Template called "Preliminary Incident Report Form"

  1. You can view previous submissions by clicking on each card to open the details of the submission. To report a new preliminary incident click on "Create Submission"

  1. Fill in the details required for the Preliminary Incident Report and scroll to the bottom and click "Submit Form"

  1. You will get a confirmation pop up with an option to submit another incident report, view submitted details or return to screen of previous submissions
